Category 1 inappropriate restraints and physical abuse (Subject 1) unsubstantiated: Subject performed proper restraint. Category 3 obstruction (Subjects 2 & 3) unsubstantiated: no first-hand knowledge.

Category 3 inappropriate restraint and neglect found when the Subject performed an unwarranted restraint and used an inappropriate technique.

2 Subjects, Category 3 neglect no breach. Category 2 restraints and physical abuse reversed. Knee strike and pressure point technique were reasonable emergency intervention.

Category 1 physical abuse affirmed. Subject admitted to unauthorized restraint, kicked/pulled chair, causing Service Recipient to fall to the floor then choking or strangling him.

Category 3 neglect and physical abuse affirmed: Subject hit Service Recipient with door. Category 2 restraints and physical abuse affirmed: Subject pushed Service Recipient's neck.
