Category 2 physical abuse, neglect, and abuse (deliberate inappropriate use of restraints) found when the Subject recklessly dragged the Service Recipient across the floor.
Category 3 abuse substantiated when Subject pinned and threatened Service Recipient. Likelihood of impairment because of Service Recipient's behavior thereafter.
Category 3 abuse found when the Subject attempted to perform a standing restraint, and pushed the Service Recipient, causing the service recipient to fall.
Category 3 abuse (deliberate inappropriate use of restraint) found when Subject failed to de-escalate and restrained the Service Recipient in violation of TCI techniques.
Category 2 abuse (deliberate inappropriate use of restraints) found when the Subject dragged the Service Recipient by the arm establishing the likelihood of injury.

Category 3 physical abuse and abuse found. Subject struck the Service Recipient in the face after being bitten, in contravention to training. Inappropriate restraint found.
