Category 2 neglect found. Subject breached duty by failing to properly supervise subordinate substance abuse counselor resulting ultimately in Service Recipient overdosing.

Category 2 neglect found. Subject’s statements were varied and inconsistent. Subject found to have been asleep or less than alert. Subject not credible.
