Category 2 Neglect substantiated. Subject failed to follow service recipient's behavioral support plan resulting in his arrest for sexual assault.

Category 2 physical abuse substantiated. Subject intentionally kicked the service recipient in the groin causing injury.

Category 3 neglect substantiated. Subject breached duty when failing to follow service recipient's dietary guidelines, resulting in choking risk.

Category 3 negelct substantiated. Subject left naked Service Recipient unstrapped in wheelchair. Category 2 neglect substantiated. Subject exited facility with service recipient naked in bath. 

Subject failed to notify facility of Service Recipient's blood test results. As a custodian, Subject was required to take action even while off duty.
