Multiple category 1 sexual abuse and category 2 neglect found. Subject forcibly touched Service Recipient. Discrepancy in Service Recipients statement was not material. 

Category 2 physical abuse found. Subject recklessly caused a physical injury to Service Recipient by holding keys in his hand while performing a restraint. 

Category 2 physical abuse and neglect found. Subject breached her duty by escalating the Service Recipient and placing her hand on the Service Recipient's neck.

Category 2 neglect found when Subject breached a duty to provide adequate medical care. Subject failed to call emergency services contrary to facility policy.

Category 2 and 3 neglect found when Subject breached duties of supervision. Subject's testimony was not credible.

Category 1 sexual abuse and Category 2 neglect found. Credible evidence that Subject forcibly touched the Service Recipient based upon expert's opinion and corroborating evidence.

Category 2 neglect found when Subject breached a duty by failing to ensure the Service Recipient's wheelchair was secure before vehicle transport. Serious injury likely.

Category 2 physical abuse, inappropriate restraints (Subject 1) and Category 3 neglect, inappropriate restraints (Subject 2) found against both Subjects where they performed restraint with excessive force.

Category 2 physical abuse found where the Subject punched the Service Recipient three times in the face and neck area with a closed fist.
