Category 2 physical abuse, deliberate inappropriate use of restraints and Category 3 neglect found. Subject shoved and held Service Recipient down on neck; failed to use de-escalation techniques. 

Credible evidence demonstrates the Subject acted recklessly by threatening to call the police on the Service Recipient and calling him the derogatory term faggot.

Category 3 negelct substantiated. Subject left naked Service Recipient unstrapped in wheelchair. Category 2 neglect substantiated. Subject exited facility with service recipient naked in bath. 

Category 3 neglect found.  Subject failed to notfy the RN when Service Recipient complained of pain by saying "ouch."

Category 3 physical abuse substantiated. Subject slapped service recipient; Category 3 neglect substantiated. Subject threatened service recipient.

Subject's defense that he had no choice but to leave Service Recipient unsupervised in a van for 13 minutes is discreditted. 

Category 3 abuse (deliberate inappropriate use of restraints) and neglect found.  Restraint was deliberate even if quickly aborted by Subject.  No emergency intervention exception.

Category 3 neglect found. The Subject failed to take the Service Recipient to the hospital as directed.
